For family reunion Visa, we have to submit a relation proof certificate (marriage or Birth certificate etc.) If there is any change in name and surname given in certificate and passport, then you have to prepare one and same certificate for each error. Even in case of parents name given in birth certificate for child, for example, in comparison between marriage certificate and child passport, you need this certificate if there is any difference. One common mistake could be giving surnames as initials in marriage certificate and full expanded surname in passport. If 2 or more errors are in one certificate (ex-birth certificate) , a single ‘one and same certificate’ mentioning all the clarifications can be produced. In Italy , if on family reunion/dependent visa after posting the Permesso application by postal ,the postal receipt and Codice fiscal are enough to get a personal doctor assigned. We can get assigned a personal pediatrician for our child by visiting the local ASL office. In india vaccination is voluntary ,not compulsory. In Italy it’s compulsory and requested in case of enrolling children in schools. Most of the Vaccines are same as norm from WHO .So if some vaccines are already completed in india , please take file(Papers) containing the schedule calendar (detailing the complete record of vaccines finished by your child). Pls Note: Some vaccines are scheduled in India, but not in Italy ( example-Typhoid prevention vaccine ,since its not common /not necessary in Italy) .And vice versa .
